I plan on spending 15-17 days in India. I would like to see Delhi, Jaipur, Agra, Varanasi, Khajuraho, Arangabad, Cochin, and Kovalam.

India is a very vast country. It is not possible to see all the places you mentioned in 15 days, especially because they are spread over India from north to south. One more thing, it is currently monsoon in India so, most parts shall have rains which make tour a little uncomfortable. I would recomend you a theme tour so that you make best out of your time.
As regards tour operator, there are a large number of reliable tour operators. Dewan tours, samsan travels, incredible india are among the best. All have their own web sites which you can easily surf.
